Medical Billing Services in Hawaii
Billing, coding, credentialing and denial management for Hawaii practices, with Medicare claims through Noridian Healthcare Solutions (Jurisdiction JE) and Hawaii Medicaid handled by people who know the rules.

Medical billing for Hawaii practices
Hawaii has an unusual payer mix. HMSA and Kaiser Permanente cover a large share of residents, the Prepaid Health Care Act pushes most working adults into employer coverage, and nearly all Medicaid members are enrolled in Med-QUEST health plans. Practices on Oahu deal with a crowded HMSA and Kaiser market, while neighbor island clinics serve more QUEST and Medicare patients and often have fewer billing staff to draw on.
SFA Solutions supports Hawaii practices remotely. Our team works Central time business hours, so your claims, denials and payer follow-up are moving before your office opens. We bill Medicare Part B through Noridian, work all five QUEST Integration plans, and keep your HOKU Medicaid enrollment and CAQH profile current so new providers can start seeing insured patients sooner.

What Hawaii billing involves
These are the programs and rules our team works with for Hawaii claims.
Medicare
Noridian Healthcare Solutions · JEHawaii Part B claims are processed by Noridian Healthcare Solutions in Jurisdiction JE. Medicare's filing limit is 12 months from the date of service, and appeals start with a redetermination within 120 days of the initial decision.
Hawaii Medicaid
Med-QUEST (Hawaii Medicaid), administered by the Department of Human Services Med-QUEST Division; QUEST Integration managed care
Portal: HOKU, the Med-QUEST online Medicaid provider application system, for enrollment and status checks; fee-for-service billing follows the Med-QUEST provider manual and bulletins
Medicaid managed care
Almost all Medicaid members are enrolled in one of five QUEST Integration plans: AlohaCare, HMSA, Kaiser Permanente, 'Ohana Health Plan and UnitedHealthcare Community Plan. Plan networks and availability can differ by island, so confirm the member's plan and your participation for each island you serve.
Timely filing
12 months from the date of service for fee-for-service claims (HAR 17-1739.1-16); QUEST plans set their own limits in provider contracts.
Commercial and Medicare Advantage
HMSA (Blue Cross Blue Shield of Hawaii) and Kaiser Permanente dominate commercial coverage, with UHA Health Insurance and HMAA also active. HMSA, Kaiser, Humana and UnitedHealthcare carry most Medicare Advantage members.
Prompt pay law
Hawaii's clean claim law, HRS 431:13-108, requires payment within 15 days for clean claims submitted electronically and 30 days for clean claims submitted on paper.
Nurse practitioners
Full practiceHawaii NPs can evaluate, diagnose and prescribe without a physician agreement, so they credential and bill under their own NPI with most plans. Confirm each payer's NP fee schedule, since some plans pay NPs at a percentage of the physician rate.
Credentialing
Every provider who sees Med-QUEST members must register in HOKU, including providers who only bill QUEST plans, and the plans check for that registration. HMSA and Kaiser each run their own credentialing and network processes, so start those applications early. Most other carriers pull from CAQH ProView, which should be re-attested every 120 days.
Billing challenges Hawaii practices face
Two dominant payers
HMSA and Kaiser together hold a large share of Hawaii's insured patients, and each has its own fee schedules, prior authorization rules and portal. A denial trend with HMSA can affect a big piece of your revenue. We track denials by payer and reason so problems show up early.
Time zone gaps
Hawaii is two to six hours behind the mainland, depending on the time zone and time of year, which leaves a short window to reach mainland payer call centers and Medicare contractors. Our team works Central time business hours, so payer calls, appeals and status checks get done while your staff focuses on patients.
QUEST plan coordination
With five QUEST Integration plans and island specific networks, billing the wrong plan or a provider who is not loaded on that island leads to denials. We verify the member's plan before the visit and confirm your participation with each plan.

Hawaii medical billing questions
What is the timely filing limit for Hawaii Medicaid?
Med-QUEST fee-for-service claims must be submitted within 12 months of the date of service under HAR 17-1739.1-16. QUEST Integration plans set their own filing limits in provider contracts, so check each plan's provider manual.
Who processes Medicare claims in Hawaii?
Noridian Healthcare Solutions is the Jurisdiction E (JE) A/B MAC for Hawaii, along with California and Nevada. Enrollment changes go through PECOS, and Noridian's portal handles eligibility and claim status.
Do I need to register in HOKU if I only see QUEST plan members?
Med-QUEST requires providers serving Medicaid members to register through HOKU, including providers who bill only the managed care plans. Missing registration can stop plan payments, so it is one of the first items we check during onboarding.
