Overview

Why general surgery billing takes specialty knowledge

Surgical billing is about what happens around the procedure as much as the procedure itself. Major surgeries carry a 90-day global period that bundles the pre-op day, the surgery and routine follow-up. Minor procedures carry 0 or 10 days. A visit that leads to the decision for surgery, a planned staged procedure, a return to the OR for a complication or an unrelated problem during the global each needs its own modifier to be paid.

SFA Solutions codes from your operative notes and clinic documentation and keeps a running record of every patient's global period. We apply 57, 58, 78, 79, 24 and 25 when the documentation supports them, code hernia repairs under the current size-based codes, bill assistant surgeons with 80, 82 or AS, and check NCCI edits before claims go out. The goal is that surgeons are billed for the work they actually documented.

Common problems

Where general surgery practices lose revenue

Global periods and the decision for surgery

A 90-day global includes the day before surgery, the day of surgery and routine post-op care. The visit where the decision for a major surgery is made is billed with modifier 57 when it falls on the day before or the day of surgery. For minor procedures with 0 or 10-day globals, a separate same-day E/M needs modifier 25.

Staged, related and unrelated procedures

Modifier 58 is used for a staged or more extensive procedure planned at the time of the first surgery and starts a new global. Modifier 78 is used for an unplanned return to the OR for a related complication and does not start a new global. Modifier 79 is used for an unrelated procedure during the global, and modifier 24 for an unrelated E/M visit during the post-op period.

Hernia repair coding

Since 2023, anterior abdominal hernia repairs (ventral, umbilical, incisional and spigelian) are coded with 49591-49618 based on initial or recurrent status, total defect size and whether the hernia is reducible or incarcerated. Mesh placement is included in these codes. Inguinal hernia repairs still use their own codes, such as 49505 and laparoscopic 49650.

Multiple procedures and assistants

When multiple procedures are done in one session, the highest-valued procedure is listed first and others carry modifier 51 where the payer requires it, with payment reductions applied. NCCI edits bundle many combinations unless a distinct site supports 59 or XS. Assistant surgeons bill with 80 or 82, and NPs or PAs assisting bill with AS, but only on procedures the payer allows an assistant for.

A/R, credentialing and payers

What else affects general surgery revenue

A/R challenges in general surgery

Surgical A/R tends to sit in a few large claims. Missing authorization on an elective case, a global period denial on a visit that was actually unrelated, or an assistant surgeon claim on a code where the payer does not allow an assistant can each hold up thousands of dollars. Delayed operative notes also slow claim submission, since nothing can be coded until the note is signed. High deductibles on elective hernia and gallbladder cases leave patient balances that are best estimated and discussed before surgery.

Credentialing considerations

General surgeons enroll under taxonomy 208600000X and need current privileges at every hospital and ASC where they operate. PAs and NPs who first assist must be enrolled with each payer to bill with modifier AS, and some commercial plans do not pay non-physician assistants at all. Locum surgeons covering call need either their own enrollment or proper locum billing with modifier Q6, within Medicare's time limit for locum arrangements.

Payer considerations

The Medicare physician fee schedule assigns each code an assistant surgeon indicator, and claims for an assistant are paid only when that indicator allows it or medical necessity is documented. Commercial plans may use different global periods or bundling logic than Medicare, so we check payer policy for high volume procedures. Workers' compensation surgeries follow the state fee schedule and usually need authorization from the adjuster, and many Medicare Advantage plans require prior authorization for elective procedures that traditional Medicare does not.

Coding and compliance

General Surgery coding details that affect payment

ICD-10 considerations

Gallbladder disease uses combination codes in K80 that capture the stone, any cholecystitis and obstruction in one code, such as K80.00 for gallstones with acute cholecystitis without obstruction. Appendicitis codes in K35 identify peritonitis, abscess and perforation, which support the complexity of the case. Hernia codes in K40 to K46 should match the CPT's recurrent or initial status and any obstruction or gangrene. Breast cancer codes in C50 carry laterality and quadrant, such as C50.411 for the upper outer quadrant of the right female breast, and wound debridement claims need ulcer codes in L97 or L89 that state site, laterality and depth or stage.

Modifiers to watch

22Used when the operative note shows substantially greater work than usual, such as dense adhesions or morbid obesity that add significant time.
54Added when the surgeon performs only the operation and another provider takes over the postoperative care under a documented transfer of care.
55Used by the provider who assumes postoperative management after a documented transfer of care, with the dates of care on the claim.
76Applied when the same surgeon repeats the same procedure later on the same day, such as a second debridement.
62Used when two surgeons of different specialties each perform a distinct part of one procedure, such as an exposure for another surgeon, where the payer allows co-surgeons.

Global periods

Laparoscopic cholecystectomy (47562) and appendectomy (44970) carry 90-day globals, while the 2023 anterior abdominal hernia codes 49591 to 49618 carry 0-day globals, so follow-up visits after those repairs are separately billable. Debridement (11042) is a 0-day code and simple incision and drainage (10060) a 10-day code.

Medicare considerations

Medicare pays a physician who assists at surgery 16% of the surgeon's fee schedule amount and an NP or PA assistant at 85% of that amount, only on codes where the assistant indicator allows it. Teaching surgeons must document presence during the critical portions of the procedure to bill with modifier GC. Panniculectomy performed in a hospital outpatient department is on Medicare's prior authorization list, so the facility's approval should be confirmed before the case.

KPIs

General Surgery billing KPIs to track

  • Global period denial rate. Visits denied as global usually mean modifiers 24, 25 or 57 are missing or the visit really was related, and both are fixable.
  • Days from surgery to signed op note. Unsigned operative notes are the main cause of surgical charge lag, and a few late notes can hold large claims.
  • Assistant surgeon claim payment rate. Assistant claims on codes without an assistant indicator are denied, so this rate shows whether codes are checked before billing.
  • Authorization match rate on elective cases. CPT codes that differ from what was authorized lead to denials on high-dollar elective claims.

General Surgery billing audit checklist

  1. Confirm hernia codes reflect initial or recurrent status, total defect size and reducible or incarcerated status from the op note.
  2. Check that decision-for-surgery visits use 57 for major procedures and 25 for minor ones.
  3. Verify assistant surgeon claims are only billed on codes the payer allows an assistant for.
  4. Review visits during global periods for 24 or 79 and documentation of an unrelated problem.
  5. Confirm debridement codes match the deepest tissue removed and the surface area documented.
  6. Match authorized CPT codes against billed codes on every elective case.

How we work

How SFA runs general surgery billing

1

We verify benefits and authorization for scheduled surgeries before the case date.

2

Operative notes are coded with the right procedure codes, modifiers and assistant claims.

3

Each patient's global period is tracked so post-op visits and new problems are billed correctly.

4

Claims are checked against NCCI edits, most clean claims are submitted within 24 hours, and denials are appealed with the operative report.

Questions

General Surgery billing questions

When should I use modifier 57 instead of 25?

Use 57 on an E/M where the decision for a major surgery (90-day global) is made, on the day before or day of surgery. Use 25 for a significant, separate E/M on the same day as a minor procedure with a 0 or 10-day global.

What is the difference between modifiers 58 and 78?

Modifier 58 is for a planned or staged procedure, or one more extensive than the first, and starts a new global period. Modifier 78 is for an unplanned return to the OR for a related complication and is paid for the intraoperative portion only.

Is mesh billed separately for hernia repairs?

For anterior abdominal hernia repairs coded 49591-49618, mesh placement is included. For open inguinal hernia repairs, mesh is not separately reported either. Check current CPT guidance for the specific repair.

Can an NP or PA bill as an assistant at surgery?

Yes, with modifier AS, on procedures the payer allows an assistant for. Medicare pays a reduced percentage of the surgeon's fee for non-physician assistants.

Are post-op visits billable?

Routine post-op visits within the global period are included in the surgical payment and billed with 99024 for tracking only. Visits for unrelated problems are billed with modifier 24.
