Medical billing for New Jersey practices

New Jersey practices bill into one of the most concentrated commercial markets in the country. Horizon Blue Cross Blue Shield of New Jersey carries a large share of commercial lives, while NJ FamilyCare members are spread across five managed care plans. Most offices along the Turnpike corridor also see New York and Pennsylvania plans walk through the door, so eligibility checks and out-of-state rules come up daily.

Medicare Part B claims go to Novitas Solutions in Jurisdiction L. NJ FamilyCare fee-for-service claims run through NJMMIS, and each managed care plan keeps its own authorization grid and appeal timelines. SFA Solutions works with practices across New Jersey remotely from Austin, inside your existing EHR, submitting most clean claims within 24 hours and working denials toward payment.

What New Jersey billing involves

These are the programs and rules our team works with for New Jersey claims.

Medicare

Novitas Solutions · JL

New Jersey Part B claims are processed by Novitas Solutions in Jurisdiction JL. Medicare's filing limit is 12 months from the date of service, and appeals start with a redetermination within 120 days of the initial decision.

New Jersey Medicaid

NJ FamilyCare (New Jersey Medicaid and CHIP), administered by the Division of Medical Assistance and Health Services (DMAHS)

Portal: NJMMIS (njmmis.com) for fee-for-service enrollment, eligibility and claims, with Gainwell Technologies as fiscal agent

Medicaid managed care

Most NJ FamilyCare members are enrolled in one of five managed care plans: Aetna Better Health of New Jersey, Fidelis Care, Horizon NJ Health, UnitedHealthcare Community Plan and Wellpoint (formerly Amerigroup New Jersey).

Timely filing

1 year from the date of service for fee-for-service professional claims (N.J.A.C. 10:49-7.2); managed care plans set their own limits by contract, generally 180 days from the date of service for original claims.

Commercial and Medicare Advantage

Horizon Blue Cross Blue Shield of New Jersey is the largest commercial carrier, with Aetna, UnitedHealthcare, Cigna and AmeriHealth New Jersey also active. Horizon, Aetna, UnitedHealthcare and Humana all offer Medicare Advantage plans in the state.

Prompt pay law

Under New Jersey's prompt pay rules, carriers must pay or deny clean claims within 30 calendar days when submitted electronically and 40 calendar days when submitted on paper (N.J.A.C. 11:22-1.5).

Nurse practitioners

Reduced practice

Under S2996/A4052, signed March 30, 2026, NPs who provide primary care or behavioral health care and have more than 5,000 hours of practice in their population focus can prescribe without a joint protocol. Other NPs still need a joint protocol with a collaborating physician. NPs bill under their own NPI, and some plans ask for the joint protocol or proof of hours during enrollment.

Credentialing

Enroll with NJ FamilyCare through NJMMIS first, then contract and credential with each managed care plan separately, since every plan requires an active Medicaid ID. Horizon, Aetna, UnitedHealthcare and Cigna pull from CAQH ProView, so an attested, current CAQH profile shortens the process. Medicaid enrollment must be revalidated at least every five years.

Common problems

Billing challenges New Jersey practices face

Out-of-network and surprise billing rules

New Jersey's Out-of-Network Consumer Protection Act limits what patients can be billed for certain out-of-network services and sends payment disputes to arbitration. Practices that are out of network with Horizon or other carriers need clean documentation and correct patient notices before any balance bill goes out.

Five Medicaid plans, five rulebooks

Aetna Better Health, Fidelis Care, Horizon NJ Health, UnitedHealthcare and Wellpoint each publish their own prior authorization lists and appeal deadlines. A service that needs no authorization with one plan can deny with another, so authorization checks have to happen per plan and per member.

Cross-border patients

Many New Jersey practices treat patients covered by New York and Pennsylvania plans or employer plans administered out of state. Claims often route to a home plan through the BlueCard program or a different claims address, and missing that step is a common cause of avoidable denials.

Questions

New Jersey medical billing questions

What is the timely filing limit for NJ FamilyCare claims?

Fee-for-service professional claims must be received within one year of the date of service under N.J.A.C. 10:49-7.2. Managed care plans such as Horizon NJ Health or Aetna Better Health set their own filing limits, and the state's provider guidance lists 180 days from the date of service for original managed care claims. Check the plan's current provider manual for each payer you bill.

How fast do New Jersey insurers have to pay clean claims?

State rules require payment or denial within 30 calendar days for electronic clean claims and 40 calendar days for paper claims. Self-funded employer plans governed by ERISA are generally outside these state rules. We track payer aging so late payments get followed up instead of sitting in A/R.

Who is the Medicare contractor for New Jersey?

Novitas Solutions processes Medicare Part A and Part B claims for New Jersey as part of Jurisdiction L. Enrollment changes go through PECOS, and claims follow Novitas local coverage determinations.

Do you work with practices outside the major New Jersey cities?

Yes. SFA Solutions works remotely from Austin with practices anywhere in New Jersey, from Bergen County to Cape May. We log in to your existing EHR or practice management system, so there is no new software to install.
