Pediatrics billing services
Well visits, vaccines, screenings and sick visits often happen in one appointment. We bill each piece so your pediatric practice is paid for all of it.

Why pediatrics billing takes specialty knowledge
Pediatric practices see high volume at modest rates, which makes small coding gaps expensive over a year. A typical well-child visit can include a preventive E/M, several vaccines with counseling, developmental screening, vision and hearing screening and a sick visit for an ear infection. Each part has its own code, and payers apply bundling rules that differ between commercial plans and Medicaid.
SFA Solutions codes each component the documentation supports, including vaccine counseling units, screening instruments and modifier 25 when a problem is addressed at a well visit. We know how Vaccines for Children stock is billed in Medicaid and how EPSDT schedules work. We work inside your current EHR, aim to submit most clean claims within 24 hours of final review and follow up on underpaid vaccine and screening lines.

| 90461 | Immunization administration through age 18 with counseling, each additional component |
| 96110 | Developmental screening with scoring and documentation, per standardized instrument |
| 96127 | Brief emotional or behavioral assessment, per standardized instrument |

Where pediatrics practices lose revenue
Vaccine administration with counseling
For patients under 19, 90460 is billed for the first vaccine component when the provider counsels the family, and 90461 for each additional component. A combination vaccine like DTaP-IPV-Hib carries several components. When counseling is not documented, 90471 and 90472 apply instead, which pays less.
Vaccines for Children stock
VFC vaccines are free to the practice, so only the administration fee is paid. Many state Medicaid programs want the vaccine CPT code billed at $0 or with modifier SL, and each state's rules differ. Billing VFC vaccine at full price causes recoupments.
Screenings that add up
Developmental screening (96110), emotional or behavioral assessments (96127), caregiver depression screening (96161), vision screening (99173) and hearing screening (92551) are separately billable with most payers. Bright Futures schedules tell you when each is expected, and missing them is a common revenue gap.
Sick and well visits together
When a significant problem is managed during a well visit, a problem E/M (99212 to 99215) can be billed with modifier 25 alongside 99381 to 99395. Some payers reduce the second visit, and Medicaid plans vary, so the documentation must clearly separate the two.
What else affects pediatrics revenue
A/R challenges in pediatrics
Pediatric A/R is shaped by Medicaid and CHIP, which cover a large share of children and bring retro-eligibility changes and frequent managed care plan switches. Vaccine lines are another source of aging, since underpaid administration codes and vaccine products paid below cost are easy to miss. Patient balances are small but frequent, so collecting at the visit makes a real difference.
Credentialing considerations
Pediatricians typically need enrollment with the state Medicaid program, each Medicaid managed care plan in the area and CHIP plans where they are separate. Practices that give vaccines to eligible children need enrollment in the Vaccines for Children program through the state health department. New physicians and pediatric nurse practitioners should be enrolled before they start seeing patients, since many managed care plans do not pay for visits before the effective date.
Payer considerations
Under ACA rules, non-grandfathered commercial plans cover recommended childhood vaccines and Bright Futures preventive services without cost sharing, but they differ on paying screenings like 96110 and 96127 separately from the well visit. Medicaid's EPSDT benefit requires states to cover medically necessary services for children under 21, even when the adult benefit does not include them. Some plans reduce payment when a problem visit with modifier 25 is billed on the same day as a well visit.
Pediatrics coding details that affect payment
ICD-10 considerations
Well-child visits use Z00.129 when no abnormal finding is made and Z00.121 when one is, with the finding coded as well, while newborns under 8 days use Z00.110 or Z00.111. Developmental screening has its own codes, such as Z13.41 for autism screening and Z13.42 for screening for global developmental delays. Otitis media codes in H65 and H66 carry laterality (H66.93 for bilateral otitis media, unspecified), and asthma codes in J45 are coded by severity and whether there is an exacerbation or status asthmaticus. Z23 is added for vaccine encounters, and ADHD claims use F90 codes by presentation type.
Modifiers to watch
| EP | Used by some state Medicaid programs on services provided under the EPSDT program. |
|---|---|
| SL | Applied to state-supplied vaccine product lines in states whose Medicaid program uses it for Vaccines for Children stock. |
| 33 | Used on commercial claims for ACA preventive services, such as screenings billed outside the well visit, so cost sharing is waived. |
| 25 | Added to the problem E/M when a significant illness is managed during a well visit, with the extra work documented. |
Global periods
Global periods rarely apply in pediatrics. Minor office procedures such as simple laceration repair carry a 0-day global, while incision and drainage (10060) carries a 10-day global, so a wound check during that window is included.
Medicare considerations
Medicare rarely applies to pediatric practices, since children qualify for Medicare only in limited situations such as end-stage renal disease. Medicaid and CHIP are the main government payers, and state Medicaid programs apply NCCI edits to physician claims, so bundling rules still matter. Children with both Medicaid and commercial coverage need the commercial plan billed first, with Medicaid as payer of last resort.
Pediatrics billing KPIs to track
- Vaccine administration units per well visit. Missing counseling components on combination vaccines lowers revenue on every immunization visit.
- Screening capture compared with Bright Futures. Comparing billed screenings to the periodicity schedule shows how much work is done but not billed.
- Medicaid managed care eligibility denials. Frequent plan switches mean claims sent to the old plan deny, so this rate shows how well eligibility is checked.
- Vaccine payment compared with acquisition cost. Private stock vaccines paid below cost lose money on every dose, so contracts need regular review.
Pediatrics billing audit checklist
- Confirm 90460 and 90461 units match the vaccine components and documented counseling.
- Check that VFC vaccines are billed at $0 or with SL according to state Medicaid rules.
- Verify screenings such as 96110 and 96127 are billed at the ages the payer expects.
- Review same-day sick and well visits for separate documentation and modifier 25.
- Confirm Medicaid managed care plan assignment is checked before each visit.
- Check that newborn claims are filed under the correct parent's plan or Medicaid once the baby is added.

Pediatrics billing questions
When can we bill 90460 instead of 90471?
90460 requires that the physician or other qualified provider counsel the patient or family about the vaccine and that the patient is 18 or younger. Without documented counseling, 90471 and 90472 are used.
How do we bill VFC vaccines?
Bill the administration code for each vaccine. The vaccine product code is reported at $0 or with modifier SL based on your state Medicaid program's rules.
Can we bill a sick visit and a well visit on the same day?
Yes, when a significant problem requires additional work beyond the preventive service. The problem E/M is billed with modifier 25 and the documentation should show the extra history, exam or decision making.
Is developmental screening separately billable?
Yes, 96110 is billed per standardized instrument with most payers. Medicaid EPSDT programs expect it at specific ages.
Do you bill newborn hospital care?
Yes. We bill normal newborn care (99460 to 99463), subsequent hospital care and discharge services, and we watch for newborns who are not yet on a parent's plan.
