Medical billing for Idaho practices

Idaho practices work in a mostly rural state with long distances between clinics and a fast growing Boise and Treasure Valley population. Medical care for most Idaho Medicaid members is still paid fee-for-service through the state, with behavioral health carved out to a separate plan and dual eligibles served by Idaho Medicaid Plus plans. A 2025 state law directs Idaho to move Medicaid to comprehensive managed care, now planned to start January 1, 2030, which will change how many practices bill.

SFA Solutions works with Idaho practices remotely. We bill Medicare Part B through Noridian, handle Idaho Medicaid claims and enrollment through the IDMedicaid portal, and work Blue Cross of Idaho, Regence and other commercial payers. Most clean claims are submitted within 24 hours, and we follow up on open balances until they are paid or resolved.

What Idaho billing involves

These are the programs and rules our team works with for Idaho claims.

Medicare

Noridian Healthcare Solutions · JF

Idaho Part B claims are processed by Noridian Healthcare Solutions in Jurisdiction JF. Medicare's filing limit is 12 months from the date of service, and appeals start with a redetermination within 120 days of the initial decision.

Idaho Medicaid

Idaho Medicaid, administered by the Idaho Department of Health and Welfare, including Medicaid expansion adults

Portal: IDMedicaid provider portal (idmedicaid.com), with enrollment and claims handled by fiscal agent Gainwell Technologies; providers first create a Trading Partner Account

Medicaid managed care

Most physical health services are paid fee-for-service. Behavioral health is managed through the Idaho Behavioral Health Plan, administered by Magellan Healthcare since July 1, 2024. Dual eligibles are served through Idaho Medicaid Plus or Medicare Medicaid Coordinated Plans offered by Molina Healthcare of Idaho and UnitedHealthcare Community Plan of Idaho. House Bill 345 (2025) requires a move to comprehensive managed care, which the Department of Health and Welfare now expects to start January 1, 2030.

Timely filing

365 days from the date of service; Medicare crossover claims within 6 months of the Medicare payment date.

Commercial and Medicare Advantage

Blue Cross of Idaho and Regence BlueShield of Idaho are the largest commercial carriers, followed by SelectHealth, PacificSource and Aetna. Humana, UnitedHealthcare and Blue Cross of Idaho lead Medicare Advantage enrollment.

Prompt pay law

Under Idaho Code 41-5602, insurers must pay or deny electronic claims within 30 days of receipt and paper claims within 45 days, when the claim was submitted within 30 days (electronic) or 45 days (paper) of service. Late claims accrue interest under Idaho Code 41-5603.

Nurse practitioners

Full practice

Idaho NPs practice and prescribe independently, with no required physician agreement. They can enroll under their own NPI with Medicare, Idaho Medicaid and most commercial plans.

Credentialing

Idaho Medicaid enrollment starts with a Trading Partner Account on idmedicaid.com, then an online application with a signed provider agreement and W-9 uploaded to Gainwell. Idaho is moving provider types it designates as high risk to revalidation every three years, more often than the federal five year minimum. Blue Cross of Idaho and Regence use CAQH ProView, so keep that profile attested every 120 days.

Common problems

Billing challenges Idaho practices face

Medicaid managed care is coming

Idaho's move to comprehensive managed care will add MCO contracts, credentialing and new prior authorization rules for practices that have billed the state directly for years. Practices that clean up their Medicaid enrollment data now will have an easier transition. We monitor state announcements and prepare your enrollment files ahead of plan launches.

Behavioral health carve out

Mental health services for Medicaid members go through the Idaho Behavioral Health Plan, while medical services go to Idaho Medicaid. Integrated practices that bill 90837 psychotherapy and 99214 visits on the same day need each claim routed to the right payer. We split these claims and route each part to the correct payer before submission.

Two Blue plans

Blue Cross of Idaho and Regence BlueShield of Idaho are separate companies with separate contracts, fee schedules and portals. Patients often carry cards that look similar, and billing the wrong Blue plan causes avoidable denials. We verify coverage before the visit.

Questions

Idaho medical billing questions

What is the timely filing limit for Idaho Medicaid?

Idaho Medicaid generally requires claims within 365 days of the date of service. Medicare crossover claims are accepted within six months of the Medicare payment or EOB date, as long as the Medicare claim itself was filed on time.

Who is the Medicare contractor for Idaho?

Noridian Healthcare Solutions processes Idaho Medicare Part A and B claims as the Jurisdiction F (JF) A/B MAC. Enrollment changes are filed through PECOS.

Is Idaho Medicaid moving to managed care?

Yes. House Bill 345, passed in 2025, directs the state to move Medicaid to comprehensive managed care. The Department of Health and Welfare has said the shift is planned for January 1, 2030, so watch its notices for contract and credentialing dates.

How fast must Idaho insurers pay claims?

Idaho Code 41-5602 requires insurers to pay or deny electronic claims within 30 days of receipt and paper claims within 45 days, as long as the provider submitted the claim within those same windows after the service. Overdue amounts carry interest under 41-5603 at the statutory rate.
