Medical billing for practices on eClinicalWorks

SFA Solutions handles billing for practices on eClinicalWorks, working inside the eCW system you already use. Your providers keep documenting in V12, and our team works claims, payments and denials on the billing side.

No EHR migration required. SFA is not affiliated with or endorsed by the platform vendor.

About billing in eClinicalWorks

eClinicalWorks V12 combines the EHR with practice management and revenue cycle technology that eCW describes as using bots to scrub and submit claims, receive and post electronic remittances, and manage denials and appeals. Its Clinical Rules Engine can drop billing codes based on documentation, and an EOB to ERA feature creates electronic remittances when a payer only sends paper. eCW lists Optum, TriZetto Provider Solutions and Waystar as partner clearinghouses. eCW also offers its own RCM service and CBO technology for billing offices that serve multiple practices.

Does SFA work with eClinicalWorks?

Yes. We work inside your eClinicalWorks system using user accounts your practice creates for our team. We are not an eClinicalWorks partner and do not have an integration with eCW; we work the same screens your billing staff would.

What billing work does SFA handle in eClinicalWorks?

We review charges and codes before release, submit claims through your connected clearinghouse, work rejections and denials, post ERA and paper EOB payments, and follow up on aging A/R by payer. We can also check eligibility before visits when your front desk wants help with it.

How does onboarding work?

It starts with a free practice audit, where we review your billing in eClinicalWorks and walk you through where revenue may be slipping. If you move forward, we sign a Business Associate Agreement, your administrator creates user accounts for our team, and we review your payer list, fee schedules and open A/R. We also confirm which clearinghouse your eCW system is connected to, since rejection reports come back from it.

Do we need to change our EHR?

No. We work inside your existing eClinicalWorks setup, so there is no migration and your providers keep documenting the way they do today. Your eCW templates, Clinical Rules Engine settings and clearinghouse connection stay as they are.

What access does SFA need?

We need eClinicalWorks user accounts with billing and payment posting permissions, plus read access to progress notes where coding review is part of the work. Your eCW administrator sets those permissions and can disable the accounts at any time.

How are claims handled?

We review codes the Clinical Rules Engine dropped against the note before release, then submit through your clearinghouse. Claims are reviewed against applicable billing, coding and payer-specific rules (10,000+ in our workflow) before submission to help identify preventable errors. Clearinghouse rejections are corrected and resubmitted, with most clean claims submitted within 24 hours of final review.

How are payments posted?

ERAs post electronically in eCW, and we work the lines that do not post cleanly, such as unmatched claims or adjustments that need review. For payers that only mail EOBs, we post them manually or through the EOB to ERA option if your practice uses it, then reconcile to the bank deposit.

How are denials worked?

We sort denials by reason code, correct what can be corrected, and appeal with the records the payer asks for. You decide on any write-off, and we keep a log of each appeal and its outcome.

How does SFA Intelligence fit in?

SFA Intelligence is our AI-assisted workflow, powered by the RCM Employee platform, which we license. It works alongside eClinicalWorks, reading the claim and remittance data your practice authorizes, to run eligibility checks (270/271), check coding against NCCI edits, scrub claims, categorize denials by reason, flag timely-filing risk in A/R and reconcile 835 payments to spot underpayments. Your team keeps using eClinicalWorks as usual, and our billers review and own every claim decision. There is no direct integration with eClinicalWorks, only the access your practice grants.

What reporting will we get?

You get monthly reporting on collections, A/R aging, denials and clean claim rate, reviewed with your named account manager. We use eCW's financial dashboards and reports where your access allows. Figures are reconciled to what eClinicalWorks shows, so your numbers and ours match.

What security controls apply?

We sign a BAA before any work begins, and our U.S.-based team is HIPAA trained. Staff systems use MFA, staff devices are encrypted, and we complete an annual HIPAA security risk assessment. Your practice creates our eClinicalWorks accounts and can change or revoke them at any time.

What does implementation look like?

After the BAA is signed and accounts are active, we can typically begin within 24 hours of receiving system access. The first weeks focus on clearing held or rejected claims, confirming ERA enrollment with your main payers and building an A/R worklist by age and filing deadline. With eCW, we also review your claim edits and recent clearinghouse rejection patterns so recurring errors are fixed once at setup. Your named account manager is your single point of contact throughout.

Billing tips for eClinicalWorks practices

Check what the rules engine dropped

The Clinical Rules Engine adds codes from documentation, which saves time but can also add a code the note does not fully support. A quick review of E/M level and add-on codes before release prevents later recoupments.

Plan for paper EOB payers

Some payers still send paper EOBs. Decide whether those are posted by hand or converted with eCW's EOB to ERA feature, and make sure one person owns them so they are not left unposted.

Read rejections at the clearinghouse level

eCW connects to partner clearinghouses such as Optum, TriZetto and Waystar. Front-end rejections (bad member ID, missing NPI, wrong payer ID) come back from the clearinghouse before the payer sees the claim, so they need fast correction to keep filing deadlines safe.
